Payment

You will be billed weekly for the previous week's care.  Bills are sent home in your child's office packet each Monday.  Payment is due in full by Friday of the week you are billed.  If your payment is delinquent by one week your child will not be allowed to use Clubhouse until your bill is paid.  You may, however, make a payment plan with Debbie Powell if you would like to pay on a twice monthly or monthly plan.  The same stipulations apply to delinquent payments.  You may also take advantage of the 10% discount plan if your child is a regular Clubhouse attender of 12 hours per week or more.

Hours of Operation

Clubhouse runs from 2:00-6:00 PM each school day unless advance notification is given.  If your child is not picked up by 6:00 PM you will be billed $20 for every 10 minutes or portion thereof that you are late.  It is essential that you or an emergency contact person can be reached at all times during our hours of operation.  If you will not be able to pick your child up at the expected time, we ask that you call the Clubhouse phone to inform the staff.  Please enter the number (508-345-4215) into your cell phone. If your child is brought to Clubhouse because he/she was not picked up on cars at the end of the school day,  you will  be billed for the first hour of Clubhouse once your child is signed in.

Behavior Expectations

In an effort to make Clubhouse a safe and enjoyable environment for all of our students, children are expected to adhere to the following behavioral guidelines:

1. Show reverence for those things considered sacred (the Bible, God's name, prayer, etc.)

2. Avoid the use of vulgarity, obscenity, and swearing

3. Show respect for self, peers, adults, and property

4. Avoid physical and verbal intimidation or attack

5. Be obedient to authority

The Clubhouse staff will take the following steps regarding disciplining a child as needed:

1.  Reminder of expectations

2.  A period of separation from play or group activity for reflection on the behavior followed by a discussion with a staff member about alternative acceptable behaviors

3. Conference with a parent

4. Discipline slip written

A child will receive a discipline slip upon the third occurrence of an unacceptable behavior in one week.  Three discipline slips will result in the child not being allowed to use Clubhouse for one week.  Three more discipline slips will result in expulsion from Clubhouse for the remainder of the school year.

Please sign and return.  In signing this form you agree to the terms found herein.
